yes that is possible, but as always: it depends.

ochCall2Action inserts the contents of a Joomla article so anything that can be done on article level, can be inserted.
So if you can show the form in a Joomla article, ochCall2action can insert it.

If you need help, I will need some more information like how do you generate the form etc.
